This page provides important information about visiting Ekaku Kids Clinic.
Please read carefully before your visit.
At our clinic, registration is done through online (WEB) reservations.
Reservations are available only on the day of your visit.
Appointments are accepted in order of registration, and specific consultation times cannot be selected.
You may choose to receive a notification email when your turn is approaching.
※ If you use a My Number Card as your insurance card, a PIN code is required.
※ Please check the expiration date.
Photos and videos can be very helpful in assessing symptoms.
If possible, please record the symptoms on your smartphone and bring them with you.
We collect personal information only within the scope necessary for medical care and treatment.
If personal information is to be used for other purposes, we will clearly inform you of the purpose and obtain your consent in advance.
We strive to keep personal information accurate and up to date and to prevent leakage, loss, damage, falsification, or unauthorized access.
When making a reservation, please check the relevant items and describe any symptoms of concern (e.g., sore throat, cough, runny nose, diarrhea, vomiting, abdominal pain, lethargy, rash).
Patients with fever (those who select “fever present”) will be examined in an isolation area.
Upon arrival, please do not enter the clinic. Call us from your car at 0565-29-0415.
When it is your turn, we will contact you from our mobile phone 080-6610-0415.
We may call you before your visit to confirm symptoms.
If your contact information has changed, if you prefer contact other than the mother, or if you have not visited our clinic in the past 3 years, please provide your contact details.
Vaccine appointments can be made through WEB reservation.

※ Vaccination records before May 2021 or vaccinations given at other clinics are not reflected.

Patients whose residence is outside Toyota City or Miyoshi City must make reservations by phone or in person.
※ A PIN code is required when using a My Number Card.
※ Please check expiration dates.
※ If someone other than a guardian accompanies the child for routine vaccinations, a letter of authorization is required.
After infectious diseases or fever, a certain interval is required before vaccination.
