Information for Patients (English)

This page provides important information about visiting Ekaku Kids Clinic.
Please read carefully before your visit.

At our clinic, registration is done through online (WEB) reservations.
Reservations are available only on the day of your visit.
Appointments are accepted in order of registration, and specific consultation times cannot be selected.
You may choose to receive a notification email when your turn is approaching.

Before Your Consultation

Please bring

• Maternal and Child Health Handbook
• My Number Card or Health Insurance Card
• Child Medical Expense Recipient Certificate

※ If you use a My Number Card as your insurance card, a PIN code is required.
※ Please check the expiration date.

Helpful items (if available)

• Notes on symptoms and fever progression / fever chart
• Extra clothes and diapers
• Photos of rashes, abnormal stools, etc.
• Videos showing seizures

Photos and videos can be very helpful in assessing symptoms.
If possible, please record the symptoms on your smartphone and bring them with you.

Important Requests Before Examination

• Please avoid eating or drinking before the consultation, as we need to examine the mouth and vomiting may occur during the examination.
• If your child has vomiting, frequent urination, or pain during urination, a urine test may be performed before the consultation. Please inform the reception staff.
• If someone other than the usual caregiver accompanies the child, bringing a memo describing symptoms and their course will help ensure a smooth consultation.
• For children under 1 year old, weight measurement may be performed as needed to assess growth and dehydration.

Privacy Policy

We collect personal information only within the scope necessary for medical care and treatment.
If personal information is to be used for other purposes, we will clearly inform you of the purpose and obtain your consent in advance.
We strive to keep personal information accurate and up to date and to prevent leakage, loss, damage, falsification, or unauthorized access.

For Patients with Fever (Please Read Carefully)

When making a reservation, please check the relevant items and describe any symptoms of concern (e.g., sore throat, cough, runny nose, diarrhea, vomiting, abdominal pain, lethargy, rash).
Patients with fever (those who select “fever present”) will be examined in an isolation area.
Upon arrival, please do not enter the clinic. Call us from your car at 0565-29-0415.
When it is your turn, we will contact you from our mobile phone 080-6610-0415.
We may call you before your visit to confirm symptoms.
If your contact information has changed, if you prefer contact other than the mother, or if you have not visited our clinic in the past 3 years, please provide your contact details.

About Vaccine Reservations

Vaccine appointments can be made through WEB reservation.
If you have any questions, please feel free to contact us by phone.

Reservation Details

• First-time visitors may also make reservations.
• Reservation period: 7 days to 1 month in advance
• Reservations are made as set packages (e.g., our recommended “2-month set”).
• Our system automatically calculates age and vaccination intervals and guides you to the appropriate timing.

※ Vaccination records before May 2021 or vaccinations given at other clinics are not reflected.
Please confirm using the Maternal and Child Health Handbook.

Vaccines Not Available for WEB Reservation

• Rotarix
• Japanese Encephalitis (2nd stage)
• Diphtheria–Tetanus (DT, 2nd stage)
• HPV vaccine
• Mumps

Patients whose residence is outside Toyota City or Miyoshi City must make reservations by phone or in person.

What to Bring on the Day of Vaccination

• Vaccination ticket (for routine vaccinations)
• Maternal and Child Health Handbook
• Patient card
• My Number Card or Health Insurance Card
• Child Medical Expense Recipient Certificate
• Pre-vaccination questionnaire (if provided)

※ A PIN code is required when using a My Number Card.
※ Please check expiration dates.
※ If someone other than a guardian accompanies the child for routine vaccinations, a letter of authorization is required.

Changes or Cancellations

After infectious diseases or fever, a certain interval is required before vaccination.
